Client Rights

Information about requesting your records, privacy, professional licensing, and filing a complaint.

Request Your Health Care Records

To request access to your records, contact Spy Creek Psychological Services at (830) 282-1380 or info@spycreek.com. Ask for the records-request process or submit a request through your established secure client portal.

Identify the records and dates of service you are requesting and your preferred delivery method. We may need to verify your identity or authority to request another person’s records and obtain an authorization before releasing information. Please use the secure portal for sensitive details rather than the public website form. Records requests are handled in accordance with applicable state and federal law; any permitted fees and limitations will be explained.

Contact the Licensing Authority

The Texas Behavioral Health Executive Council (BHEC) oversees the Texas licensing boards for psychologists, professional counselors, social workers, and marriage and family therapists.

File a Professional Complaint

You may submit a concern about a licensed provider directly to BHEC. Visit the complaint instructions linked above, select “How to File a Complaint,” and follow the Council’s current submission instructions. You do not need permission from our practice to contact the licensing authority.

File a Consumer Complaint

You may also file a consumer complaint with the Office of the Texas Attorney General, Consumer Protection Division. Review the instructions, select the appropriate complaint form, and provide the business name, relevant dates, a description of the concern, and supporting documents as appropriate.
